This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Who We Are (ACE Designation)
This notice describes the privacy practices of the entities participating in the Athena Certification Center LLC Affiliated Covered Entity (ACE), including:

These designated entities, which are under common ownership and control, have formed an ACE to comply with HIPAA. ACE participants may share your Protected Health Information (PHI) with each other as needed to carry out Treatment, Payment, and Health Care Operations (TPO).
Your Rights: Taking Control of Your Information
When it comes to your health information, you have certain rights. To exercise any of the rights below, contact our Privacy Office via email: support@startjourneymeds.com.
Get a Copy of Your Medical Record: You can ask to see or get an electronic or paper copy of your medical record. We will provide a copy or summary, usually within 30 days. We may charge a reasonable, cost-based fee.
Correct Your Medical Record: You can ask us to correct information you think is incorrect or incomplete. We may say "no," but we will tell you why in writing within 60 days.
Request Confidential Communications: You can ask us to contact you in a specific way (e.g., home vs. office phone) or to send mail to a different address. We will say "yes" to all reasonable requests.
Ask Us to Limit What We Share: You can ask us not to use or share certain info for treatment, payment, or operations. We are not required to agree, unless you pay for a service out-of-pocket in full; in that case, we will say "yes" to withholding that info from your health insurer.
Get a List of Disclosures: You can ask for a list (accounting) of the times we’ve shared your info for six years prior to the date you ask, who we shared it with, and why (excluding treatment, payment, and operations).
Choose Someone to Act for You: If you have medical power of attorney or a legal guardian, that person can exercise your rights and make choices about your health information.
File a Complaint: You can complain if you feel we have violated your rights. We will not retaliate. You can also file with the U.S. Dept. of Health and Human Services (HHS).
Your Choices: When You Have the Say
For certain health information, you can tell us your choices about what we share.
Your Preference Matters: Talk to us if you have a preference for sharing info with family/friends, in disaster relief, or in a hospital directory. If you are unable to tell us (e.g., unconscious), we may share info if we believe it is in your best interest.
Written Permission Required: We never share your info for marketing, the sale of your info, or most psychotherapy notes without your explicit written authorization.
Fundraising: We may contact you for fundraising, but you have the right to opt out at any time.
Our Uses and Disclosures
We typically use or share your health information in the following ways:
Treat You: We share info with other professionals treating you (e.g., a specialist).
Run Our Organization: We use info to manage your treatment and improve our practice. We use Artificial Intelligence (AI) and machine learning to analyze health info to identify ways to improve services and clinical workflows.
Bill for Services: We share info with health plans to get payment.
Public Good & Legal Requirements
We are allowed or required to share your information for:

Public Health/Safety: Preventing disease, product recalls, or reporting suspected abuse/neglect.

Research: Conducting health research.

Comply with Law: If state or federal laws require it.

Special Requests: Organ donation, workers’ compensation, law enforcement requests, or responding to lawsuits/subpoenas.
Our Responsibilities

We are required by law to maintain the privacy and security of your PHI.

We will let you know promptly if a breach occurs that may have compromised your info.
